1. Schedule a New Patient visit with one of our providers. Make an appointment by calling (716) 297-2052.

2. Print and complete the Registration Forms.

3. Prepare for your office visit – Bring your insurance card, payment, photo identification, list of current medications, and vaccination record.

4. Please arrive 30 minutes before your scheduled appointment if you have your new patient forms filled out, and 60 minutes before your scheduled appointment if you do not already have the paperwork filled out.
